Over-the-Scope Clip Closure of Oesophageal Perforation after Pneumatic Dilatation for Achalasia

چکیده
Achalasia is an esophageal disease due to inability of relaxation of the lower esophageal sphincter (LES), frequently associated with loss of peristalsis in the distal esophagus results from the degeneration of ganglion cells in the myenteric plexus in the esophageal wall [1,2]. Pneumatic dilatation (PD) of the esophagus is the first line method of therapy of achalasia. Pneumatic balloon dilatation induce forceful stretching of the lower esophageal sphincter, which weakens the LES by tearing its muscle fibers [3,4].

Risk factors of oesophageal perforation during pneumatic dilatation for achalasia.
BACKGROUND/AIMS Pneumatic dilatation of the oesophagus is a well established treatment for achalasia. Oesophageal perforation is the most serious complication that occurs in 2% to 6% of cases. The aim of this retrospective survey was to identify predictive risk factors for perforation in a consecutive series of 218 patients with achalasia. METHODS Between 1983 and 1993, 270 pneumatic dilatati...
متن کاملBotulinum toxin versus pneumatic dilatation in the treatment of achalasia: a randomised trial.
BACKGROUND Intrasphincteric injection of botulinum toxin is a new treatment option for achalasia. AIMS To compare the immediate and long term efficacy of botulinum toxin with that of pneumatic dilatation. METHODS Symptomatic patients with achalasia were randomised to botulinum toxin (22 patients, median age 57 years) or pneumatic dilatation (20 patients, median age 56 years). Symptom scores...
متن کاملComplete endoscopic closure (clipping) of a large esophageal perforation after pneumatic dilation in a patient with achalasia.
The risk of esophageal perforation following endoscopic balloon dilation for achalasia is in the range of 1%-5%, with a mortality rate of 1%-20%. Perforations need to be recognized early, and, if reasonable, an immediate endoscopic repair should be pursued quickly. Herein, we report a case of successful endoscopic closure by clipping of a large iatrogenic perforation in a patient with achalasia...
متن کاملEndoscopic closure of transmural esophageal perforation after balloon dilation for achalasia.
A 56-year old woman was transferred to our hospital 1 day after balloon dilation for recently diagnosed achalasia that had been complicated by transmural esophageal perforation. Physical examination revealed tachycardia and fever, as well as left-sided pleural fluid. Emergency surgery was performed, which involved transhiatal primary closure of a 4-cm long transmural perforation, debridement of...
متن کاملTwenty four hour oesophageal acidity in achalasia before and after pneumatic dilatation.
Radiotelemetric ambulatory 24 hour oesophageal pH measurement was carried out in 17 patients with symptomatic manometrically proven achalasia before and after pneumatic dilatation. Before dilatation an abnormally high percentage acid exposure time was present but typical episodes of gastro-oesophageal reflux occurred in only one patient. In nine patients who had a oesophageal food residue these...
